Given the current situation it is difficult for patients to attend for their annual long term condition review as they would normally.
If you are due for an Asthma review, we will be contacting you by text in the first instance with a link to a very short questionnaire for you to complete and return to us by text. It is a really short questionnaire and should only take a minute or two to complete. Once returned this will be reviewed by our Practice Nurse who will contact you if any further information is needed.
This will mean that your review is not delayed, and if you are on any medication this can be updated at the same time.
If you do receive a text about your asthma review, please take a couple of minutes to fill it in and text it back to us. Even if your asthma is very mild, you don’t take any medication and don’t usually come in for a review, it would be really helpful if you could do this.
